Dulaglutide: Medicine Overview
Dulaglutide (Trulicity) is a once-weekly GLP-1 receptor agonist built as a fusion protein; this overview covers label indications, the REWIND heart trial and main safety information.

What dulaglutide is
Dulaglutide is a good example of why "peptide" is not one tidy category. The US label describes it as a fusion protein: two identical, disulfide-linked chains, each with a GLP-1 analog sequence joined by a short peptide linker to part of a modified human antibody (IgG4 Fc), made in hamster-cell culture. [1] Its GLP-1 part is 90% homologous to natural GLP-1(7-37). [1] Because of the antibody part, its half-life is about 5 days. [1] Whether you call it a large peptide or a small protein, the useful question is what it was approved for; see peptides vs proteins.Evidence labels used: approved medicine (a regulator reviewed it for a named use). The brand is Trulicity; dulaglutide is the active ingredient. See active ingredient, brand, formulation and indication.
Formulation
The US label lists Trulicity as a solution in single-dose pens, available in several strengths. [1] Strength and device are part of the product, so they are not interchangeable with other once-weekly products that use a different molecule and device, such as semaglutide pens. Brand, ingredient and form are separate ideas; see the guide linked above.Mechanism
Dulaglutide activates the GLP-1 receptor on pancreatic beta cells, raising cyclic AMP and glucose-dependent insulin release; it also lowers glucagon secretion and slows gastric emptying. [1] The biology is covered in GLP-1 and GIP biology. What the trials showed
- Heart events (REWIND). In a cardiovascular outcomes trial of 9,901 people with type 2 diabetes (4,949 on dulaglutide, 4,952 on placebo), major heart events occurred in 12.0% versus 13.4% (hazard ratio 0.88, interval 0.79 to 0.99). Median follow-up was 5.4 years. [1]
- Components. Non-fatal stroke was lower (hazard ratio 0.76). Cardiovascular death (0.91) and non-fatal heart attack (0.96) had intervals that included no effect. [1]
- Children. A 26-week placebo-controlled trial in 154 patients aged 10 and older supports the pediatric diabetes use in the US. [1]
- Against tirzepatide. Dulaglutide served as the comparison drug in SURPASS-CVOT, where tirzepatide met non-inferiority but superiority was not established. See the tirzepatide overview. [3]
Label: the REWIND result is the main heart evidence, in a type 2 diabetes population only.
Care context
Dulaglutide is prescription-only. Changes in treatment belong with a prescriber, and nothing here is advice to start, stop or switch. For diabetes in general see prediabetes, type 2 diabetes and metabolic syndrome, and for other medicines in the class see metabolic and diabetes peptide medicines.Frequently asked questions
Is Trulicity the same as Ozempic?
No. Both are once-weekly GLP-1 receptor agonists, but they are different molecules from different companies, each with its own label. [1][4]
Is dulaglutide approved for weight loss?
The US label I read lists blood sugar control and heart-event reduction, not weight reduction. [1]
Did dulaglutide reduce heart attacks?
In REWIND the combined outcome was lower, but the non-fatal heart attack component alone was not clearly different from placebo. [1]
Is it approved for children?
The US label covers type 2 diabetes from age 10, and the EMA page lists patients 10 and older. [1][2]

Approved uses (read 2026-10-22)
- US (FDA label). Improve blood sugar control in adults and children 10 and older with type 2 diabetes; and reduce the risk of major heart events (cardiovascular death, non-fatal heart attack, non-fatal stroke) in adults with type 2 diabetes who have established heart disease or multiple risk factors. [1]
- EU (EMA page). Patients 10 and older with insufficiently controlled type 2 diabetes, as monotherapy when metformin is unsuitable or alongside other diabetes medicines. [2]
- Weight. Weight reduction is not among the indications listed on the US label I read. [1]
Main safety information from the label
- Boxed warning. In male and female rats dulaglutide increased thyroid C-cell tumors; whether it does in humans is unknown. It is contraindicated with a personal or family history of medullary thyroid carcinoma or MEN 2. [1]
- Common effects. Nausea, diarrhea, vomiting, abdominal pain and decreased appetite (5% or more of people). [1]
- Warnings list. Pancreatitis (discontinue if suspected), low blood sugar when combined with insulin or sulfonylureas, kidney injury from dehydration, severe stomach and bowel reactions, serious allergic reactions and gallbladder disease. [1]
- Antibodies. In diabetes trials of 24 to 104 weeks, 64 of 3,907 treated adults (1.6%) developed antibodies against dulaglutide. See immunogenicity of peptide medicines. [1]
- Pregnancy. Data are limited; animal studies suggest possible fetal risk, and the label says use in pregnancy only if benefit justifies risk. [1]
