Psychedelic Clinical Trials and Mental Health Research: Findings and Limits
What clinical trials of psilocybin and MDMA-assisted therapy have found for depression and PTSD, why blinding and expectancy limit them, and what regulators have decided so far.

Quick answer
Early trials of psilocybin for depression and MDMA-assisted therapy for PTSD have shown large effects in some studies. They are small, short and hard to blind, and they take place in tightly controlled settings with screened participants. In the US the FDA has not approved MDMA-assisted therapy after reviewing the first application, and it has published rules for how psychedelic trials should be run. This page describes research, not treatment advice. Evidence labels: promising but limited, mixed, insufficient evidence, approved medicine (none found for these uses in the sources read). How psychedelics act in the brain (neuroscience)
NIDA says classic psychedelics such as psilocybin, LSD and DMT mainly affect 5-HT2A receptors, which are normally activated by the chemical messenger serotonin. [1] Research on why this might help mood is ongoing; findings from brain imaging or animals are preclinical or biomarker evidence and do not by themselves show a treatment works. See history and neuroscience.Psilocybin and depression: what trials found
| Trial | Design | Main finding |
|---|---|---|
| Goodwin 2022 [2] | Phase 2, treatment-resistant depression, 233 adults, three single-dose groups with psychological support | The 25 mg group improved more than the 1 mg control at week 3; the 10 mg group did not. Adverse events in 77% of participants, and suicidal thoughts or self-injury in all dose groups. |
| Raison 2023 [3] | Phase 2, major depression, 104 adults, psilocybin vs niacin placebo | Larger drop in depression scores than placebo at day 43; more adverse events, including more severe ones, but no serious treatment-related events. |
| Carhart-Harris 2021 [4] | Phase 2, 59 adults, psilocybin vs the antidepressant escitalopram | No significant difference on the main measure at 6 weeks; many secondary outcomes favored psilocybin but were not corrected for multiple comparisons. |
| Davis 2021 [5] | 27 adults, immediate vs waiting-list treatment | Used a waiting list instead of a placebo, so participants knew whether they had been treated. |
Label: promising but limited. These studies were small to mid-sized, one was funded by a drug company [2], and participants were selected. Trial amounts are study amounts, not recommendations.
Participants in modern studies are screened: NIDA notes people with diagnosed psychosis or bipolar disorder, and sometimes other mental illnesses, are excluded as a precaution. [1] The JAMA trial also excluded active substance use disorder and active suicidal ideation with intent. [3]
Psychedelic-assisted therapy in serious illness
A 2024 Cochrane review of six randomized trials (149 participants with life-threatening illness) found classic psychedelics may reduce anxiety, but judged certainty low to very low because of high risk of bias and small samples. It reported no treatment-related serious adverse events. [6]MDMA-assisted therapy and PTSD
NIDA says some people consider MDMA to have psychedelic properties, and it is studied alongside them. [1] In a phase 3 trial of 90 people with severe PTSD, MDMA with therapy lowered PTSD scores more than placebo with therapy. [7] In August 2024, however, the FDA issued a complete response letter, declining approval at that time. Its reasons included concern about the reliability of the safety data, no proof the effect lasts beyond 18 weeks, and that about 40% of enrolled participants reported prior MDMA use. [8] The letter also described functional unblinding and expectation as problems to handle in any new study. [8] We did not verify later events, so check the FDA for current status.Research limitations (blinding and expectancy)
- Blinding. Participants usually know when they received a drug with strong effects. This is called functional unblinding. The FDA says it can lead to expectation bias in people who feel perceptual effects, and that therapists and raters may also guess. [9] The Cochrane review notes that blinding could not be achieved in all six studies. [6]
- Expectancy. If people hope a treatment will work, they may report improvement. The FDA suggests measuring expectations, checking whether blinding held, and using central raters who do not know the treatment given. [9]
- Therapy versus drug. The FDA states that the contribution of psychotherapy to the overall effect is not established. [9]
- Selection and setting. Screened participants, trained staff and monitored sessions do not match how people use these substances outside trials. NIDA notes poor mental health outcomes may be more likely outside clinical use. [1]
- Durability and safety. The FDA says long-term, blinded follow-up and good tracking of all adverse events, including euphoria-related ones, are needed. [9][8]
Regulatory status
- United States. The FDA finalized guidance for companies running psychedelic trials in July 2026. [9] NIDA states that two psilocybin formulations received Breakthrough Therapy designation, which is not an approval. [1] MDMA-assisted therapy was not approved after the 2024 review. [8] A search of the FDA documents read found no approved psilocybin product, but we did not check every FDA page; verify with the FDA.

Where this leaves readers
Research is real and promising for specific groups, in clinics, with screening. It does not support self-treatment. For safety see adverse effects and contraindications and drug interactions and toxicology. For depression or trauma, talk with a clinician: talking with a health professional. In the US, the 988 Suicide & Crisis Lifeline is available by call or text to 988.Frequently asked questions
Is psilocybin therapy FDA approved?
We found no approved psilocybin product in the FDA documents read, only trial guidance and a Breakthrough Therapy designation. Check the FDA for updates. [9][1]
Did psilocybin beat an antidepressant in trials?
In one 59-person trial it did not significantly beat escitalopram on the main measure. [4]
Why do trials say "functional unblinding"?
Because participants can usually tell whether they got a psychedelic, which can inflate results through expectation. [9]
Why did the FDA decline MDMA-assisted therapy in 2024?
Reasons in the letter included safety data reliability, durability of effect and prior MDMA use among participants. [8]
Can these studies be applied to home use?
No. Trials used screened participants, supervision and controlled products, and outcomes outside clinics can differ. [1]
