Emergency signs first
Anaphylaxis is a severe allergic reaction that can start within seconds or minutes. MedlinePlus lists symptoms that include trouble breathing, wheezing, hives, swelling of the face, eyes or tongue, dizziness, vomiting and unconsciousness. [1] It says to call 911 or the local emergency number immediately, and that an epinephrine injector or intranasal epinephrine should be used right away if one is available. [1] Do not wait to see whether it passes. If you have a history of serious reactions, carrying epinephrine as your provider instructs is part of that plan. [1]The short answer
Collagen supplements come from animal tissue: fish skin and scales, cattle hides and bones, pigs, chicken, or eggshell membrane. A person allergic to the animal may react to the collagen. The best-documented problem is fish collagen in people with fish allergy. For shellfish, egg-derived and beef-derived collagen supplements, we found little or no direct evidence. See collagen types and sources. How the evidence is labeled
Case report means one person's reaction, which shows it can happen, not how often. Laboratory finding means antibody tests, not a confirmed reaction. Proper evidence not found means we searched and found no usable study.Source by source
| Source | What we found | Strength |
|---|---|---|
| Fish (marine) | In a 2020 report, a 9-year-old girl had anaphylaxis after two fruit-flavored gummy tablets; an oral challenge confirmed fish collagen as the cause. [2] Other case reports describe anaphylaxis to fish collagen in a gummy candy and to hydrolyzed fish collagen. [3] [4] | Case reports |
| Fish, lab data | In one study, 3 of 10 patients with fish allergy had IgE antibody to fish gelatin (type I collagen). [5] A 2022 paper proposes fish collagen as a possible indicator of severe reactions in fish allergy. [6] | Laboratory and clinical series |
| Jellyfish | A 2018 letter calls jellyfish collagen an allergen. [7] | Letter only |
| Shellfish | Proper evidence not found. We searched PubMed for collagen from crustaceans or shellfish and for marine collagen and shellfish allergy, and found no usable report. | None |
| Egg (eggshell membrane) | Proper evidence not found for allergic reactions to eggshell-membrane products. One 12-week trial of 88 adults with knee osteoarthritis called its product safe; the sponsor funded the authors. [8] | Insufficient |
| Beef (bovine) | The authors of a 2000 study wrote that bovine type I collagen is a major allergen in bovine gelatin allergy. [5] Two children had life-threatening anaphylaxis to a bovine gelatin matrix used in spinal surgery; tests were positive for beef and bovine collagen. [9] Reactions to oral beef-collagen supplements: Proper evidence not found. | Case reports; not oral supplements |
| Pork (porcine) | The same surgical report found antibodies to porcine collagen. [9] Oral supplement reactions: Proper evidence not found. | Case report |
Fish, shellfish and the label
Marine collagen is usually described by its animal source, such as fish. In the US, fish and crustacean shellfish are separate items among the nine major food allergens named by the FDA, and eggs and milk are also on the list. [10] The FDA lists them separately, and we found no source linking the two for collagen. One authors' discussion states that marine collagen can trigger an allergic reaction in people allergic to seafood. [11] Ask an allergist what applies to you.Beef, pork and the alpha-gal question
Alpha-gal syndrome is an allergy to a sugar found in mammal meat. A 2021 study notes that it can also be found in products containing gelatin of bovine or porcine origin, and that gelatin-containing vaccines caused basophil activation in the patients tested. [12] Whether hydrolyzed collagen powders react in people with alpha-gal syndrome: Proper evidence not found. If you have this condition, ask your allergist before using any beef- or pork-derived product.Cross-reactivity between animals
In the 2000 study, there was cross-reactivity among gelatins from various fishes, but little between fish and bovine gelatins. [5] This is a small laboratory study and does not replace testing.Labels can be wrong or incomplete
A 2024 analysis of 10 collagen supplements found that the labeled origin matched the identified origin for most. One product listing marine, bovine and chicken collagen most likely contained no marine collagen, and one gave no origin. [11] The authors say they found discrepancies in labeling and origin with possible safety implications. [11] The authors of the gummy report warn that processed foods may contain fish collagen. [2] See reading a collagen label. Why trials rarely show allergy
A bone trial excluded people with an allergy to collagen. [13] So "no adverse events" in such studies says little about allergic people.Frequently asked questions
Can fish collagen cause anaphylaxis?
Yes, case reports document it, including in a child after eating gummy tablets containing fish collagen. [2]
Is collagen safe if I am allergic to shellfish?
Proper evidence not found either way for collagen. Ask an allergist.
Can eggshell membrane cause an egg allergy reaction?
Proper evidence not found. We searched PubMed for eggshell membrane allergy and found no usable report.
Does beef collagen cause problems in people allergic to beef?
Bovine collagen is described as an allergen in bovine gelatin allergy. [5] Oral supplement reaction data: Proper evidence not found.
What should I do if my lips swell or I struggle to breathe after taking collagen?
Call 911 or the local emergency number. Use epinephrine if you have it. [1]
