Adaptogens for Burnout and Chronic Stress: Claims Checked

Adaptogens are often sold for burnout. This page checks what burnout means, what small trials of rhodiola and ashwagandha found, and what the evidence cannot yet say.

Safety first. Natural products can cause side effects and interact with medicines. Read who should be careful, by situation before you use anything on this page.

> If you feel hopeless, or are thinking about suicide: in the US, call or text 988 (the Suicide & Crisis Lifeline) or call 911 in a life-threatening situation. [1] The 988 Lifeline is described as available around the clock, free and confidential. [2] A herb is not a substitute for care from a clinician when exhaustion and low mood last.

The claim as it is marketed

Products say that adaptogens (herbs thought to help the body cope with stress; see What are adaptogens?) help you "recover from burnout" or "reset" after long-term stress. This page asks what burnout actually is, and what people who studied it found.

What "burnout" means

The World Health Organization's ICD-11 lists burn-out as an occupational phenomenon, and says it is not classified as a medical condition. [3] It defines it as a syndrome resulting from chronic workplace stress that has not been successfully managed, with exhaustion, mental distance from the job, and reduced professional efficacy. [3] A German health technology assessment noted that the definition and diagnosis of burnout are scientifically criticized. [4] Burnout can overlap with depression, anxiety and physical illness; see Stress, anxiety, burnout and depression: how they differ and Burnout: signs and recovery.

What the studies found

QuestionWhat the source foundStrength of evidence
Does Rhodiola rosea root extract help stress-related fatigue?A placebo-controlled trial of 60 adults, selected by Swedish criteria for fatigue syndrome, ran 28 days. Both groups improved on a burnout scale; the extract group improved more on that scale and some attention measures. [5]Promising but limited (one small, short trial of one extract)
Does rhodiola help people with burnout symptoms?A 12-week open-label trial of 118 outpatients had no comparison group; most measures improved over time. [6]Insufficient evidence (open-label, no control)
How do burnout treatments compare overall?A review of 17 studies found enough studies to prove efficacy only for cognitive behavioural therapy, and also reported considerable natural recovery in some studies. [4]Established for CBT; rhodiola mentioned as one study
Does Withania somnifera (ashwagandha) root extract help chronic stress?A trial of 64 adults with a history of chronic stress (60 days) found lower stress scores and lower serum cortisol than placebo. [7]Promising but limited (single center; people with chronic stress, not burnout)
What does the U.S. National Institutes of Health say?For rhodiola, not enough reliable evidence for any health purpose; for ashwagandha, some preparations may be effective for insomnia and stress. [8][9]Mixed

Reviews of rhodiola for fatigue describe the research as contradictory, with studies at high or unclear risk of bias. [10] Another review noted a lack of independent replications of single studies. [11]

What a reader can safely do

Talk to a clinician about persistent exhaustion, low mood or sleep problems, because treatable causes exist. Look at workload and recovery first; the stress guides cover everyday approaches. If you still want to try a product, check the label and plant part with a pharmacist.

Frequently asked questions

Can adaptogens cure burnout?

No source we read says so. One small rhodiola trial found improvement over placebo on a burnout scale, but the evidence is limited and the review of burnout therapies found enough studies only for cognitive behavioural therapy. [5][4]

Is burnout a medical diagnosis?

The WHO's ICD-11 calls it an occupational phenomenon and says it is not classified as a medical condition. [3]

Does ashwagandha lower cortisol and fix chronic stress?

In one 60-day trial of adults with a history of chronic stress, an ashwagandha root extract lowered stress scores and serum cortisol versus placebo. [7] NIH says some preparations may help stress but many studies were small and used different preparations. [9]

Is "adrenal fatigue" the cause of burnout symptoms?

The Endocrine Society says there is no scientific proof that adrenal fatigue is a true medical condition. [12]

When should I see a clinician?

When exhaustion, low mood or sleep problems keep going, or interfere with daily life. NIMH describes depression as an illness that can cause severe symptoms affecting sleep, eating and work; if you have thoughts of suicide, call or text 988. [1]

Limits and uncertainty
  • Cortisol is a lab marker, not a diagnosis. A change in cortisol or a cortisol response does not show that a person recovered. The trials above also reported questionnaires, which are self-reported and can improve with a placebo; in the rhodiola trial both groups improved. [5]
  • "Adrenal fatigue" is not a recognized diagnosis. The Endocrine Society says no scientific proof supports it as a true medical condition, and that symptoms like tiredness can come from adrenal insufficiency, depression, sleep apnea or other problems. [12] See Adrenal fatigue and adrenal support claims.
  • Results belong to one extract. A result for one rhodiola extract or one ashwagandha root extract does not transfer to other products.
  • Funding is not described in the abstracts we read.
Safety and who should ask first
Rhodiola side effects reported by NIH include dizziness, headache, insomnia and dry mouth or extra saliva, and an interaction with losartan (a blood pressure medicine) has been reported. [8] NIH also says depression can be a serious illness and to consult a health care provider. [8] For ashwagandha, NIH lists drowsiness and stomach upset, rare liver injury cases, avoidance in pregnancy and breastfeeding, and caution before surgery or with autoimmune or thyroid disorders. [9] NIH says it might interact with medicines for diabetes and high blood pressure, sedatives, anti-seizure medicines, thyroid hormone medicines and immunosuppressants. [9] See adaptogen medicine interactions and how interactions work. Ask the prescriber or pharmacist before combining any herb with a medicine.
