CoQ10 and Statin Muscle Symptoms: What Trials Found

Does coenzyme Q10 ease muscle pain from statins? Five meta-analyses of randomized trials disagree, and the NCCIH says overall evidence does not support it.

Quick answer
Meta-analyses of randomized trials disagree on whether coenzyme Q10 (CoQ10) eases muscle pain in people taking statins. Some found less pain, others found no clear benefit, and NCCIH concludes that the overall evidence does not support it (page updated 2019). [1] If you have muscle pain on a statin, the safe first step is to tell your prescriber. Do not stop or change the statin on your own. The NHS says that if you think your medicine is causing side effects, you should talk to a doctor, who will check what is causing the symptoms and may prescribe another statin or change the dose. [2]

How the terms are used

"Statin-associated muscle symptoms" covers pain, weakness, cramps or tiredness. Trials measure them with pain scores, and sometimes with a blood marker of muscle injury (creatine kinase). A blood level of CoQ10 is a different thing: a 2018 meta-analysis found statin treatment lowers circulating CoQ10, [4] but that lab result does not show that muscle symptoms are caused by low CoQ10 or that supplements fix them.

What the reviews found

ReviewWhat it includedWhat it found
2015, Mayo Clinic Proceedings6 studies, 302 people on statins [5]No significant effect on muscle pain despite a trend toward a decrease. [5]
2018, Journal of the American Heart Association12 randomized trials, 575 people [6]CoQ10 ameliorated muscle pain and other muscle symptoms; no reduction in plasma creatine kinase. [6]
2020, Atherosclerosis7 trials, 321 people with statin-associated muscle pain; 37 to 76 people per trial [7]No benefit for myalgia compared with placebo, and no difference in staying on the statin; only two trials showed a positive effect. [7]
2022, Irish Journal of Medical Science8 studies, 472 people; 4 studies with 220 people measured muscle pain [8]No benefit over placebo for muscle pain. [8]
2025, Journal of Nutritional Science7 trials, 389 people, lasting 30 to 90 days, with study amounts of 100 to 600 mg per day [9]A significant reduction in pain intensity overall: four trials significant, three not. The authors say more research is needed. [9]

Study amounts are what researchers gave in trials, not advice.

Why the results differ

The trials are small (37 to 76 people per trial in the 2020 review, 35 to 76 in the 2025 review) and short (30 to 90 days in the 2025 review), and each review pooled a different set. [7][9] The 2020 review, for example, counted only trials of oral CoQ10 versus placebo in adults with statin-associated myalgia. [7] Combining small studies can swing either way, and the 2022 and 2025 reviews reached opposite conclusions on the main question, so a clear answer is not available. Evidence label: mixed.

A related finding that should not be transferred

In a review of heart failure trials, statin use did not appear to change CoQ10's effect on heart outcomes. [10] That is a different population and outcome and says nothing about muscle pain.

Frequently asked questions

Should I take CoQ10 if my statin causes muscle pain?

That is a decision for you and your prescriber. Reviews disagree, and NCCIH says the overall evidence does not support it. [1] Do not stop the statin on your own. [2]

Can I just stop my statin and see?

Please talk to the prescriber first. They can check the cause and may change the statin or dose. [2] MedlinePlus notes pain may go away on a different statin. [3]

Do statins lower CoQ10 in the blood?

A 2018 meta-analysis of 12 trials found they do, compared with placebo. [4] This is a lab measure, not a symptom.

Did CoQ10 help people stay on their statin?

A 2020 meta-analysis found it did not improve the proportion remaining on statin treatment. [7]

Which review is right?

We cannot say. Results conflict, trials are small, and the newest review calls for more research. [9]

Why this matters, and when to get help quickly
Muscle pain is one of the common side effects listed for statins. [2] MedlinePlus says muscle damage with statins is rare, but in rare cases it can lead to muscle pain and kidney damage, and a provider may order a blood test if you have muscle pain. [3] Severe or worsening muscle pain or weakness, or anything that feels serious, needs prompt medical advice, not a supplement trial.
Interactions and safety
Drugs that lower cholesterol, blood pressure or blood sugar may decrease the effects of CoQ10, and CoQ10 may change how the body uses warfarin and insulin. [11] Mild effects such as insomnia or digestive upset can occur. [1] Pregnancy, breastfeeding, children, kidney disease and liver disease are not covered for CoQ10 in these sources, so ask a clinician or pharmacist.
