Berberine: Evidence for Blood Sugar, Cholesterol and Weight - What Trials Found

What reviews of berberine trials found for blood sugar, cholesterol and weight, how big the effects were, and why lab numbers and trial quality limit the conclusions.

Quick answer
Reviews of randomized trials found that berberine lowered blood sugar and LDL cholesterol on average, and results for body weight were mixed. All of these are lab or body measurements, not heart attacks, strokes or diabetes complications, and the reviews repeatedly ask for better-quality trials. [1] This page describes research. It is not advice to use berberine, and diabetes and high cholesterol need a clinician. Never replace or stop a prescribed diabetes or cholesterol medicine because of a supplement.

How to read the table

Evidence labels: "promising but limited" (several trials agree, but size or quality is a problem), "mixed" (reviews disagree), "insufficient evidence" (nothing usable found). Study amounts and products differed between trials and are not recommendations. Most trials here were short, between weeks and months, and a change in HbA1c or LDL is a biomarker, not proof of fewer complications.

Blood sugar

QuestionWhat the source foundLabel
Fasting glucose and HbA1c in type 2 diabetes37 studies (3,048 people): fasting glucose lower by about 0.82 mmol/L and HbA1c by about 0.63 percentage points; the effect was related to starting levels. [2]Promising but limited
Same question, another review46 trials: HbA1c lower by about 0.73 when berberine was used alone or with usual diabetes care. [3]Promising but limited
Berberine versus diabetes medicinesA 27-trial meta-analysis found no statistically significant difference between berberine and oral diabetes drugs. [4] This does not show they are equivalent: see berberine vs metformin.Insufficient evidence
Long-term outcomes (complications, deaths)Not reported in the reviews we read.Insufficient evidence

In the larger glucose review, hypoglycemia (low blood sugar) was not significantly more common, but some people were also taking other diabetes drugs. [2] That is why anyone on diabetes medicine should tell their clinician before adding berberine. More: prediabetes, type 2 diabetes and metabolic syndrome.

Cholesterol

QuestionWhat the source foundLabel
LDL cholesterol versus placebo18 placebo-controlled trials, 4 to 24 weeks: LDL lower by about 0.46 mmol/L; gastrointestinal side effects tended to be more frequent with berberine, and no serious events were reported. [5]Promising but limited
Compared with other supplementsA network meta-analysis of 131 trials ranked bergamot and red yeast rice as the most effective for LDL and total cholesterol. [6]Mixed
Heart-disease patients44 trials in 4,606 people; the authors said results may be limited by the quality of existing research. [7]Promising but limited
Combination productsA meta-analysis found the biggest effect for berberine combined with silymarin; effects of combinations must not be assumed for berberine alone. [8]Mixed

Weight

QuestionWhat the source foundLabel
Body weightOne review of 12 studies found about 2.07 kg lower weight on average. [9] A 2026 review of 23 articles found about 0.88 kg lower. [10]Mixed
Berberine or barberryA review of 12 trials found no significant change in BMI. [11] A dose-response review found no significant decline in body weight. [12]Mixed

The 2026 review noted that trials need better reporting of purity, potency and amounts. [10] Averages of about one to two kilograms are small and do not tell us who would benefit. For wider context see weight-loss supplements and fat burners: claim check.

Why the evidence is limited

  • The umbrella review of 11 meta-analyses flagged weak methodological quality. [1]
  • Trial products differ (plain berberine, combinations, different salts), so one result should not be applied to another product.
  • Short trials cannot show long-term safety. See side effects and interactions.
  • Back to the overview: berberine.

Frequently asked questions

Does berberine lower HbA1c?

On average, yes in reviews of trials in type 2 diabetes: about 0.63 to 0.73 lower. [2] [3] Whether that prevents complications was not shown in these reviews.

Does berberine lower cholesterol?

Placebo-controlled trials found lower LDL on average. [5] Other supplements ranked higher in a network meta-analysis. [6]

Does berberine help with weight loss?

Results are mixed: some reviews found small average losses and others found no significant change. [9] [11]

Can I use berberine instead of my diabetes or cholesterol medicine?

No. The reviews did not show equivalence, and stopping a prescribed medicine should only happen with your prescriber.

Why do reviews disagree?

They include different trials, products and durations, and reviewers flagged quality problems. [1]
