TCM for Type 2 Diabetes: Evidence and Safety

What trials and reviews of specific Chinese herbal products and berberine found for type 2 diabetes and prediabetes, with low blood sugar risks and why medicines must not be replaced.

Do not stop or reduce diabetes medicines, insulin or monitoring because of any herbal product or traditional treatment. Some herbs are studied precisely because they may lower blood sugar. A review of herbal medicines for type 2 diabetes said careful monitoring for low blood sugar is essential when they are used with antidiabetic drugs, because data on herb-drug interactions are lacking. [1] Low blood sugar can cause shaking, sweating, confusion and, if severe, seizures or loss of consciousness. [2] If someone with diabetes is unconscious or having a seizure, call emergency services (911 in the US). Tell your diabetes team about every herb, tea and supplement you use: see talking with a health professional.

What this page covers

This page reports what research found for named interventions, not for "TCM" as a whole. In Traditional Chinese Medicine (TCM), a person is described by a traditional pattern, and some Chinese trials enroll people by pattern, for example "qi-yin deficiency" in trials of the granule product Jinlida. [3] These patterns are traditional frameworks, not blood tests; see traditional pattern differentiation. The question here is simpler: when a specific product was compared with placebo or usual care, what happened to blood sugar and safety?

The quality problem

A 2019 review of randomized trials in prediabetes, diabetes and its complications by authors at a Chinese TCM hospital concluded that TCM could improve glucose control, but it also said that many TCM studies have design defects such as limited duration, small samples and inconsistent efficacy measures. [4] NCCIH, the US government center for complementary health, says studies of Chinese herbal products for a range of conditions have had mixed results and that many studies were of poor quality. [5] A 2025 umbrella review (a review of reviews) of herbal medicines for type 2 diabetes found low to very low quality among the included reviews, so confidence in the findings was limited. [1]

What specific products showed

QuestionWhat the source foundStrength of evidence
Does a 10-herb capsule (Tianqi) reduce progression from impaired glucose tolerance to diabetes? In a double-blind placebo trial of 420 people over 12 months, 18.18% of the Tianqi group and 29.32% of the placebo group developed diabetes; no severe adverse events occurred. [6]Promising but limited (one trial, needs repeating)
Does Jinlida granule add to usual diabetes treatment?Of 22 trials, only 3 had low risk of bias; pooling them, HbA1c fell by 0.283 percentage points, but fasting and after-meal glucose did not change significantly. [3]Promising but limited (small effect, low-bias trials few)
Does berberine lower blood sugar?Across 37 trials with 3,048 patients, fasting glucose fell by 0.82 mmol/L and HbA1c by 0.63 percentage points, without a significant rise in total adverse events or hypoglycemia. [7]Promising but limited (many trials are small and short)
Do whole Chinese herbal formulas prevent complications? In kidney disease from diabetes, herbal products reduced albuminuria in placebo trials, with moderate to low quality evidence. [8] See TCM for kidney and liver conditions.Mixed to promising but limited

Memorial Sloan Kettering's herb reference says berberine may help with diabetes, but that studies are of limited quality and lifestyle changes are also needed. [9] Berberine is a compound found in plants used in Chinese medicine such as Coptis (huang lian). The plant is covered in Coptis and berberine, and the supplement in the berberine guide.

Safety and quality

  • Medicine interactions. Berberine has laboratory signs of affecting how sulfonylurea diabetes medicines are processed; the clinical importance is not known. It may also affect immunosuppressant drugs. [9] Details by medicine type are in Chinese herbs with diabetes and blood pressure medicines.
  • Common side effects of berberine. Appetite loss, upset stomach, diarrhea, constipation and rash. [9]
  • Pregnancy and breastfeeding. Memorial Sloan Kettering says berberine should be avoided because it may worsen jaundice in infants. [9] See pregnancy and breastfeeding.
  • Contamination and wrong herbs. NCCIH reports Chinese herbal products found to contain undeclared drugs, heavy metals, pesticides or incorrect herbs, some of which have caused organ damage. [5] See undeclared drugs in Chinese patent medicines.

Where evidence is missing

Proper evidence not found for: Chinese herbs preventing heart attacks or deaths in people with diabetes (the reviews above measured blood sugar and lab markers), or for any herbal product replacing metformin, insulin or other prescribed treatment. I checked PubMed for Cochrane reviews and placebo-controlled trials of Chinese herbal medicine in type 2 diabetes and found the reviews cited here, not long-term outcome trials.

Practical steps

Keep your usual treatment and glucose checks. If you want to try a Chinese herbal product, bring the actual package to your diabetes clinician or pharmacist before starting, and report any shakiness, sweating or confusion promptly. For prediabetes and lifestyle basics, see prediabetes and type 2 diabetes. More uses are summarized in TCM evidence by use.

Frequently asked questions

Can Chinese herbs replace diabetes medicine?

No trial reviewed here tested replacing prescribed treatment. The reviews looked at herbs added to usual care or compared with placebo, and the reviewers advised monitoring for low blood sugar when herbs are combined with diabetes drugs. [1]

Does berberine work for type 2 diabetes?

A meta-analysis of 37 trials found lower fasting glucose and HbA1c with berberine. Reference sources note that studies are often of limited quality. [7][9]

Can herbs prevent diabetes in people with prediabetes?

One double-blind placebo trial of the Tianqi capsule found fewer people developed diabetes over 12 months. It is a single trial and needs confirmation. [6]

Why are many Chinese diabetes trials hard to rely on?

Reviews point to short duration, small samples and inconsistent outcome measures. [4]

What are signs I should get urgent help?

Confusion, fainting, or a seizure in a person with diabetes needs emergency care. [2]
