Nerve pain needs a cause found first
Peripheral neuropathy happens when nerves in the hands, feet and arms are damaged. [1] Symptoms can include numbness, tingling, burning or shooting pain and weakness. [1] Diabetes is the most common cause, and high blood sugar over time can damage the nerves. [1] The NHS says to see a GP early, because the sooner it is diagnosed, the better the chance of limiting damage. [1] It adds that nerve pain is often treated with prescribed nerve pain medicines, since standard painkillers often do not work. [1]Acupuncture, if used, is an add-on. Traditional acupuncture is explained through qi and channels rather than nerve damage (see What Is Acupuncture?). This page reports what trials measured. Evidence labels: promising but limited, mixed, insufficient evidence. For sciatica and radiating back pain, see Acupuncture for Sciatica and Radiculopathy.
Summary by condition
| Condition | What the source found | Strength of evidence |
|---|---|---|
| Neuropathy of various causes | A 2017 review of 15 reports (mostly randomized trials) found most trials favoured acupuncture for diabetic neuropathy, Bell's palsy and carpal tunnel; insufficient evidence for idiopathic neuropathy. [2] | Mixed: designs varied and the reviewers asked for more sham-controlled studies. [2] |
| Painful diabetic neuropathy | A 2023 meta-analysis of 25 trials (1,561 people) found acupuncture plus usual treatment reduced pain more than usual treatment alone. [3] | Promising but limited: add-on trials with limitations. [3] |
| Diabetic neuropathy, nerve tests | A 2025 network meta-analysis of 62 trials (5,942 people) ranked electroacupuncture best for motor nerve conduction speed, with low certainty. [4] | Low certainty. [4] |
| Chemotherapy-induced neuropathy (CIPN) | A 2024 network meta-analysis of 33 studies (2,027 people) found acupuncture-related treatments better than usual care, medication or supplements for symptoms, pain and quality of life. [5] | Promising but limited; one small sham-controlled trial was inconclusive (below). |
| Shingles pain and postherpetic neuralgia | Reviews found possible benefit, mostly from trials in China comparing with drugs. [6][7] | Promising but limited to insufficient. |
Diabetic neuropathy
The trials in the 2023 review combined acupuncture, acupoint injection and moxibustion with conventional treatment, so the results show benefit as an add-on, not alone. [3] The authors noted limitations and said future studies are needed to confirm the findings. [3] The 2025 analysis restricted to people also taking mecobalamin and reported low certainty. [4] Blood sugar control remains the main way to protect nerves, and the NHS also lists stopping smoking and cutting down alcohol. [1] See a doctor for a cut or ulcer on the foot that is not getting better. [1] See also Acupuncture for Diabetes and Metabolic Health.Chemotherapy-induced neuropathy
In a 2020 US trial, 75 people with moderate to severe CIPN were randomized to real acupuncture (24), sham acupuncture (23) or usual care (21). Pain fell by 1.75 points with real acupuncture versus 0.19 with usual care at week 8, while sham acupuncture fell by 0.91 points, with a confidence interval that included no change. Numbness fell by almost the same amount with sham (1.52) as with real acupuncture (1.54). Adverse events were few and mild. [8] The authors described it as a pilot with a small sample, one centre and short follow-up. [8] That means real acupuncture beat no treatment, but the trial could not show it beat sham. For comparison, the American Society of Clinical Oncology (ASCO) 2020 guideline update said duloxetine is the only agent with appropriate evidence for established painful CIPN, though the benefit is limited, and no agent is recommended for preventing it. [9] Talk with your oncology team before any acupuncture, especially about low blood counts and infection risk; do not stop or change cancer treatment on your own. See Acupuncture in Cancer Care.
Shingles and postherpetic neuralgia
Shingles is an infection that causes a painful rash. [10] Postherpetic neuralgia (PHN) is pain that remains after the rash has healed. [7]- Urgent first: the NHS says to try to see a pharmacist within 3 days of the rash appearing, because medicine works best when started early, and to seek urgent help if the rash is on the eye or nose or if vision changes. [10]
- During shingles: a 2021 review of 21 trials (acupuncture versus antiviral therapy) reported less pain and fewer cases of PHN, and a 2022 review of 11 trials reported a lower PHN rate but no clear pain relief. The reviewers said high-quality trials were needed. [11][12]
- For PHN: a 2018 review found seven trials, all in China and all against drugs, and said results were not conclusive given low quality. [6] A 2019 review rated moderate-quality evidence for lower pain intensity but called for larger studies. [7]
Label: promising but limited; sham-controlled trials are scarce in these reviews.
Frequently asked questions
Does acupuncture help diabetic neuropathy?
Reviews of mostly add-on trials report less pain, with limitations. It does not replace blood sugar control or prescribed care. [3]
Does acupuncture help neuropathy from chemotherapy?
One small trial found improvement versus usual care, but sham acupuncture also helped, so benefit beyond expectation is unproven. [8]
Can acupuncture prevent postherpetic neuralgia?
Some reviews of trials against antivirals report fewer cases, but trial quality is limited. Shingles needs prompt medical treatment. [11][10]
Is acupuncture safe if my feet are numb?
Take extra care and tell the practitioner, because you may not feel an injury. Discuss it with your doctor first.
What should I do first for tingling or numbness?
See a GP or doctor; the NHS says earlier diagnosis improves the chance of limiting damage. [1]
